Your Specific Responsibilities:

The primary responsibility of this position is to assist in the coordination and documentation of health education messaging, community outreach events and activities as outlined in the CHP workplan.

  • Assist in the coordination and documentation of outreach events and community spotlights submissions.
  • Disseminate information to the appropriate partners regarding, resources, events, grant opportunities, health care access and quality, best practices, etc. monthly.
  • Maintain documentation of requested and disseminated resources.
  • Attend and participate in the meetings and coalitions that serve the community.
  • Increase the number of community partners and/or collaborations in projects.
  • Update Coalition members on CHD health messaging projects, events, issues, partner recruitment, and data tracking.
  • Maintain a professional and effective working relationship with external organizations that address health disparities.
  • Maintain resource guide of local agencies and organizations.
  • Update FDOH-Nassau Community Health SharePoint Events Calendar
  • Conduct planning and preparation for our reach events. Prepare weekly outreach after action reports.
  • Attend conference calls, trainings, and webinars regularly for professional development.
  • Lead or co-lead at least 4 quarterly (CHD internal) meetings, activities, or events.
  • Coordinate/participate in activities/events that support the improvement of resident’s health status.
  • Track priority area projects goals, objectives, activities in the FDOH Clear Point tracking system and update data monthly.
  • Serve on one (1) internal committee such as Employee Wellness Committee, Workforce Development, Performance Management Council, Customer satisfaction or Safety committees.

Required Knowledge, Skills, and Abilities:

  • Knowledge of the methods of compiling, organizing, and analyzing data.
  • Skill in Microsoft Office software (Word, Excel, PowerPoint, Outlook) and computer navigation.
  • Skill in identifying community related needs and how to engage and refer to those needs.
  • Ability to use various communication channels to provide education/training to audiences of varying literacy and comprehension levels.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish and maintain effective relationships with others.
  • Ability to learn and navigate PIMs computer application.
  • Ability to plan, organize, and evaluate minority health needs.
  • Ability to learn and remain in compliance with all Florida Department of Health policies and internal operating procedures and Florida Statutes while completing job duties.
  • Ability to process and handle confidential information with discretion.
  • Ability to reach, pull, and lift a minimum of 25 pounds.

Qualifications :

Minimum:

 

  • Bachelor’s degree in Health Promotion, Health Education, or Public Health or other health-related degree
  • 1 year professional experience in providing health education services.
  • Intermediate experience with Microsoft Office (i.e. Microsoft Outlook, Word, Excel, PowerPoint)

Preferred:

  • Knowledge of principles and processes for providing customer and personal services including needs assessments techniques, quality service standards, alternative delivery systems, and customer satisfaction evaluation techniques.
  • Knowledge of instructional methods and training techniques including curriculum design principles, learning theory, group and individual teaching techniques, design of individual development plans, and test design principles.
