We are looking for a Care Navigator Nurse (RN or LPN) who will help ensure patients receive the care they need to achieve incredible health outcomes and guide them along their journey to live a healthier life. These highly collaborative clinicians work closely with fellow Nurses, Providers, Health Coaches and Patient Services professionals to go the extra mile and deliver a “wow”-worthy patient experience. Patient Population: This specific Care Navigator should have experience serving patients with infectious diseases such as Hepatitis, HIV, etc.
What you’ll do in the role: - Travel to/from Community Health Center (CHC) locations and other care sites to meet our patients where they are.
- Prepare exam rooms for patient care by ensuring the room is properly stocked and cleaned.
- Room patients, check vital signs, and assess patient needs to communicate with the provider.
- Assist with patient procedures as directed by the provider.
- Communicate effectively both written and orally with patients, patients’ families, coworkers, and other outside entities; Complete routine forms for patients as appropriate.
- Ensure chart documentation in the Electronic Health Record (EHR) is correctly completed in a timely manner.
- Perform lab tests as directed by the provider, prepare specimens, operate lab equipment, and maintain laboratory documentation in accordance with CLIA standards and practice specific protocols.
- Reconcile billing from offsite labs by comparing daily lab log with patient EHR.
- Schedule offsite diagnostic tests and communicate effectively with patients.
- Facilitate referral processes for patients as needed.
- Take manual inventory of drugs.
- Monitor patient phone calls, receive and route test results for provider review, and collect data regarding patient medications; Perform front office duties as needed.
- Adhere to all Community Health Center (CHC) policies and procedures.
- Other responsibilities as necessary to meet evolving business needs.
Education & Licensure Requirements: - Licensure: Active license in good standing to practice in the State of Ohio as a Licensed Practical Nurse (LPN) required; Registered Nurse (RN) preferred.
- Education: Associate’s degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.
- Certification: Active American Heart Association Basic Life Support Provider (BLS) or willingness to obtain after joining AndHealth required; Relevant clinical specialty certifications preferred.
Other Skills or Qualifications: - Proven clinical experience required; patient navigation/care coordination/case management experience preferred.
- Prior experience serving patients with infectious diseases such as Hepatitis, HIV, etc.
- Excellent verbal and written skills. Comfortable communicating with providers and partners, and patients with varying levels of healthcare across multiple mediums daily, with a natural ability to simplify complex clinical concepts for patients with varying levels of healthcare literacy.
- Strong administrative/technical skills; Comfort working on both Mac and PC computers using Microsoft Office (Outlook, Word, Excel, PowerPoint), Google Workspace (Gmail, Google Docs/Sheets/Slides, etc.) IM/video conferencing (Slack, Teams, and Zoom), and telephones efficiently.
- Healthcare Technology: Ability to proficiently navigate and quickly learn multiple Electronic Medical and Health Record (EMR/EHR) systems.
- Trustworthy and accountable behavior, capable of viewing and maintaining confidential information daily.
Pre-employment healthcare requirements: - PPD - Tuberculosis (TB) Skin Test; Proof of negative TB skin test within the last 12 months.
- MMR - Measles, Mumps and Rubella or antibody blood titer proving immunity required.
- Varicella - (2) documented doses or antibody blood titer proving immunity required.
- Hepatitis B3 vaccination series required
- Current Influenza vaccination required.
- COVID-19 vaccination required.
