Job Title: Home Care Licensed Practical Nurse CWR
Location: Wall Township, NJ 07753
Duration: 20+ Weeks

Education:

• High School Diploma (HSD) required.

• Excellent customer service skills.
• Certified Home Care experience preferred.
• Required clinical experience with patients, including but not limited to: foley catheter management, wound care, management of lines/drains, post-operative care, and chronic disease management.

Responsibilities:

• Demonstrate knowledge and competency in performing assigned treatments and procedures according to established standards and policies.
• Follow the plan of care as directed by the Registered Nurse (RN) and/or clinical supervisors.
• Comply with physician orders and ensure that patient needs are observed, reported, and met as per the care plan.
• Record vital signs, intake/output, and other related patient information.
• Support and reinforce patient education and discharge planning.
• Assist patients with personal hygiene, nutrition, and daily activities.
• Properly utilize and maintain technical equipment and supplies.
• Assist physicians with medical treatments, exams, and surgical procedures as appropriate.
• Report changes in the patient's condition, including changes in vital signs or blood sugar levels, in a timely and accurate manner.
• Adapt work schedule as needed to meet the unit's staffing requirements and ensure adequate coverage through willingness to float.
• Participate appropriately in emergent situations, offering assistance and reassurance to patients and families.
• Complete assignments accurately and within scheduled timeframes, including data collection, care planning, and patient interventions.
• Ensure clinical practice adheres to the Code of Ethics, Board of Nursing regulations, state health department regulations, Joint Commission (JCAHO) standards, and institutional policies and procedures.
• Perform other duties and projects as assigned.
• Adhere to organizational competencies and standards of behavior.
